1. Print this Referral Form as many times as you want.
2. Fill in your name, and pass on to others.
3. When someone who has never shopped at Brookside Barkery & Bath brings the form to any location and makes a purchase, we credit your account with an extra 250 bonus points, and they get the points for their qualifying purchase that day, as well as an extra 250 bonus points to get them started!
